Can weight-loss medicines reduce heart attack or stroke risk?

Weight-loss medications are sometimes discussed as if their only outcome is a smaller number on the scale. But the FDA approved a specific cardiovascular-risk indication for Wegovy injection in March 2024, based on an outcomes trial that looked at heart attack, stroke and cardiovascular death.

Who was studied?

The large SELECT cardiovascular study enrolled more than 17,600 adults with established cardiovascular disease and overweight or obesity, without diabetes. Participants also received standard medical care and lifestyle counseling.

What happened in the study?

A major adverse cardiovascular event—cardiovascular death, nonfatal heart attack or nonfatal stroke—occurred in 6.5% of people assigned semaglutide compared with 8.0% assigned placebo over the study follow-up. The relative reduction was about 20%, while the difference in absolute event rates was 1.5 percentage points.

Does that mean everyone gets a 20% reduction?

No. The reported relative risk reduction summarizes outcomes across the studied population. It is not an individual risk prediction, and the study focused on people with pre-existing cardiovascular disease.

Is this the same approval for every weight-loss medicine?

No. FDA indications are product-specific and may depend on formulation, medical history, and evidence. Do not assume an outcome proven for Wegovy injection is automatically established for every GLP-1 medicine.

What does it mean when comparing treatments?

Ask about your cardiovascular history, other risk factors, and the exact drug indication. A choice between treatments should take more than the average weight-loss percentage into account.

The bottom line

Wegovy injection has evidence supporting a specific cardiovascular-risk reduction indication in adults with established cardiovascular disease and overweight or obesity.
