Does weight loss on GLP-1 medicines include muscle loss?
Losing body weight does not mean losing only body fat. A 2025 body-composition analysis of the SURMOUNT-1 study used DXA scans to measure fat mass and lean mass in people receiving tirzepatide. The results help readers understand what a bathroom scale cannot show.
What did researchers measure?
The researchers analyzed 160 people with DXA measurements at the start and at week 72, a subset of the full SURMOUNT-1 trial. DXA estimates body composition, including fat and lean mass; lean mass is not the same thing as muscle alone.
How much fat and lean mass changed?
Average total weight fell 21.3%, fat mass 33.9%, and lean mass 10.9% with tirzepatide in this substudy. These are separate percentage changes from different starting quantities and cannot simply be added. Results describe the study subgroup rather than every patient.
Is losing lean mass the same as becoming weaker?
Not necessarily. DXA lean mass includes water and other fat-free tissues, not just skeletal muscle. The scan did not directly measure strength or determine whether every individual experienced a functional decline.
How can people protect their health?
Ask your care team about appropriate resistance exercise, adequate food and protein intake, and strength or functional monitoring—especially if you are older, frail, or eating very little. Protein targets and exercise plans should be individualized.
What should you take away?
Focus on health, strength and sustainable nutrition alongside the weight trend. A fast drop on the scale alone cannot describe what is happening to body composition.
The bottom line
Trial data show that both fat and lean mass can decrease. Lean-mass changes are not a direct measure of muscle function.
