REDEFINE 4 CagriSema misses primary comparison objective
Source context: Read the linked original research or registry entry for study status and limitations.
Novo Nordisk published results from an open-label trial comparing CagriSema with tirzepatide. MyDosePro reviewed the originating organization's statement to identify the reported results and their limitations.
What was REDEFINE 4 designed to answer?
Novo Nordisk compared its investigational fixed-dose cagrilintide/semaglutide combination, CagriSema 2.4/2.4 mg, with tirzepatide 15 mg in an open-label Phase 3 study. The trial randomized 809 adults with obesity and at least one comorbidity and assessed outcomes after 84 weeks.
What did Novo Nordisk report?
The company reported a 23% mean weight reduction with CagriSema over 84 weeks. However, REDEFINE 4 did not achieve its prespecified primary objective of demonstrating noninferiority on weight reduction compared with tirzepatide 15 mg. The main conclusion therefore cannot be summarized only by the 23% figure.
Why does noninferiority matter?
A noninferiority study tests whether a new treatment is not unacceptably worse than the comparator under a prospectively specified statistical margin. A large weight-loss result in one arm does not establish that this comparative standard was met. Open-label design also means participants and investigators knew which treatment was assigned.
Can this tell us which medicine to choose?
Not by itself. The combination was investigational at the time of the announcement; prescribing choices must reflect currently approved products, individual medical history and safety. Even valid trial averages do not predict an individual response.
What should readers watch next?
Look for the full protocol, detailed safety outcomes, peer-reviewed publication and subsequent regulator decisions. Those are more informative than isolated comparison headlines.
The bottom line
Both the results and the failed primary endpoint should be reported.
